Who We Are
Connections365 is a nonprofit organization based in Salem, Oregon dedicated to serving youth, adults and families with complex behavioral and mental health needs.
Since 1989, we have worked alongside state agencies, families, and community partners to provide structured, compassionate care for individuals navigating significant challenges.
Today, our focus is on delivering accessible mental health services while maintaining specialized programs for those who require a higher level of support.

What We Do
Connections365 provides community-based mental health services that support youth, adults, and families through therapy, behavioral support, and coordinated care.
We also provide short term and long term residential programs for young men involved in the juvenile justice system.
These services are designed to:
- Improve emotional regulation
- Address behavioral challenges
- Strengthen family systems
- Support long-term stability
Behavioral Rehabilitation Services (Specialized Programs)
Connections365 operates structured residential programs in partnership with the state of Oregon for youth with higher-acuity needs.
We offer two distinct programs:
Short Term Residential
A short-term stabilization program focused on addressing immediate behavioral challenges and preventing further system involvement.
This program is one of only two programs of its kind in the State of Oregon.
Long Term Residential
A longer-term residential program designed to support sustained behavioral change and transition planning.
This program is one of the only models of its kind in Oregon.

- Are transitioning between levels of care
